Application forms
Most of our applicants use the OUAC application form (external link, opens in new window) .
Some exceptions apply. Please see the TMU Application and TMU International Application for more information.
Note: You must create an OUAC Account if you don’t already have one. You can only have one OUAC Account, so if you created one previously, be sure to use it. The OUAC has a base application fee which covers the initial three university/program choices. You can direct any questions about this application to the OUAC.
Use this form if:
- You're a current Toronto Metropolitan University (TMU) student
- You're a previous TMU student (since fall 1984)
- You're only applying to a part-time program at TMU
- You're applying to be a special student
- You're applying for re-admission or re-instatement
- You're a current or previous student at the Chang School or the Yeates School of Graduate and Postdoctoral Studies
The TMU Application has a fee of $90 payable via OUAC. This fee covers, and is limited to, two program choices on the application.
Readmission/reinstatement: If you received an academic standing of "Required To Withdraw (RTW)" in the last 24 months, you must request reinstatement through your program department. Changing programs: If you want to transfer to a different program (e.g. Business Management to Biomedical Sciences) you must submit a TMU Application (external link, opens in new window) and TMU Supplementary Form. Information on how previously completed TMU courses will apply toward your new program is available on the Internal Course Transfers page.
Changing plans (majors): If you are a current undergraduate student in a program that offers a choice of program plans (majors), you may be able to request a plan change (e.g. BA English to BA Sociology). The MyServiceHub Support website provides detailed instructions on how to change your plan/major.
If you are an international student, please apply using the TMU International application form. For further details visit International: How to apply.
Note: If you are an international student at an Ontario secondary school applying for September, you must use the OUAC application (external link, opens in new window) .
How to make changes to your application (e.g. address, telephone number, add/drop program choices, etc.):
OUAC application: If you applied on an OUAC application, log in to the OUAC (external link, opens in new window) to amend your application.
TMU Application/International Application: If you applied on a TMU application, complete the (PDF file) TMU change form. This form is available for use after your initial application has been acknowledged by TMU.
Supplemental information
The supplementary form is not available to the following applicants:
- Current Ontario secondary school students
- Applicants for special student status
We encourage all other applicants to submit a supplementary form. You may submit a form for each program you applied to. This form provides you with an opportunity to tell us more about yourself, including your academic history and why you are interested in the programs you have selected.

Here’s how to access and submit your supplementary form and other documents:
- Get your TMU Student Number in the email acknowledgment of your application.
- Activate your TMU online identity. Fill in the activation form via TMU accounts. When asked for your student/applicant ID, enter your nine-digit TMU Student Number. Keep a record of your user name and password.
- 24 hours after activating your TMU online identity, find the supplementary form via the ChooseTMU applicant portal.
