Google Forms is updating its interface, the revamp matches the the updates seen in the Drive, Docs, Sheets and Slides app in the last year. This fresh new interface will give you more space at the top of your form but also more control of the design and look of your custom header. 

Custom headers in the new Google Forms interface are still 800 by 200 pixels but they are no longer cropped when uploaded and you can upload transparent images.

For a tour of the new interface, while in a Google Form you are editing, go to the small question mark (?) icon at the bottom right and select “Take a tour” from the menu. For help with Google Forms, please see the Google Forms training and help page (external link, opens in new window) .
